Cancellation & Refund Policy for SlimWell Solutions Telemedicine Monthly Weight Loss Membership

Membership Cancellation

You can cancel your SlimWell Solutions telemedicine monthly weight loss membership at any time. To initiate the cancellation, you must provide a 30-day notice. To cancel:

Refund Policy

As a general rule, we do not offer refunds for our monthly weight loss membership. To avoid being charged for the upcoming month, ensure you provide the necessary 30-day notice before cancellation.

Exceptions to the Policy

Pre-Appointment Cancellations

If a patient cancels before their appointment and provides at least a 72-hour notice (3 business days), a full refund will be processed. The patient’s status will automatically be set to ‘inactive’ in the medical record system, ensuring no further charges.

Short-Notice Cancellations

If a patient cancels within 72 hours of the appointment (3 business days), the fee will not be refunded. However, the patient won’t be charged for the following month. Membership will be cancelled effectively, and the patient’s status will be marked ‘inactive’ within the medical record system.

No-Show Policy

If a patient fails to attend their appointment without providing a 72-hour notice (3 business days) to reschedule, they will be subjected to a continuous charge of the monthly membership fee. This will persist until the patient gives proper 30-day notice to cancel the membership.

Medication prescriptions are facilitated during routine monthly visits. Pharmacists dispense the medication. The membership fee applies regardless of whether the medication is prescribed or dispensed. Patients bear the responsibility to schedule, reschedule, and attend appointments. The practice will not assume responsibility for missed appointments.

By signing up for SlimWell Solutions telemedicine monthly weight loss membership, the patient agrees to the terms outlined above. It’s essential to understand and accept these terms before committing to the service.

Patient Responsibility

It is the patient’s responsibility to ensure that:

Sufficient funds are available in the associated bank account.

The credit card information on file is up-to-date and accurate, including expiration dates and CVV codes.

Policy for Declined Credit Card


This policy outlines the steps and procedures SlimWell Solutions will follow in the event a patient’s credit card is declined during the monthly membership fee processing. Our priority is to maintain transparent communication with our patients and to ensure continued access to our services.


If a patient’s credit card is declined, SlimWell Solutions will send an email notification to the patient at the email address provided upon registration. This notification will inform the patient of the declined transaction and request immediate resolution.

Payment Retrial

SlimWell Solutions will attempt to process the payment again within three (3) business days of the initial decline.

Suspension of Services

If the payment is not successfully processed after the second attempt, SlimWell Solutions may temporarily suspend the patient’s membership and access to services until the payment issue is resolved.


Once payment has been successfully processed, the patient’s membership and access to services will be reinstated.


Should a patient’s credit card be declined for two (2) consecutive months, SlimWell Solutions reserves the right to cancel the patient’s membership. Re-enrollment will require the patient to settle any outstanding balance and may be subject to additional re-enrollment fees.

SlimWell Solutions reserves the right to update this policy as necessary. Any changes will be communicated to all active members.
